Massage Business License Addendum Application
This form is a required addendum to standard business license forms, applicable to Commercial, Home Occupation, and other types of Business License Applications, for businesses offering massage, massage-like services, and massage related services.
As of September 12, 2023 there are no application fees associated with this addendum.
There are background check fees associated with certain parties to the application and the business.
